You only need a doctor’s note if you are unable to work and are ill for longer than seven calendar days.
Your employer will provide you with an SC2 self-certificate form for shorter periods of illness or alternatively you can download the form here https://www.tax.service.gov.uk/fill-online/statutory-sick-pay-employer-notification?_ga=2.63931119.154840079.1696422238-1866327464.1694166914
Following this period a doctor’s note is required. Such requests for sick notes should be made during morning surgery.
PLEASE NOTE – If you have been discharged from hospital and are not fit to return to work then it is the responsibility of the discharging hospital consultant to provide you with a fit note covering you for the expected length of time off required they deem to be necessary. (You may find you will need to remind your consultant that you require a fit note for work upon your discharge).
To make a Sick note request for yourself or anyone you care for we now ask for all requests to be made via the Ask My GP app.
